In conjunction with the numerous primary care services offered, MHC also provides an integrated array of community resources and special programs including: case management, health education, Women, Infant and Children (WIC), family planning, community health outreach & education, Tennessee Breast and Cervical Cancer Early Detection and Prevention (TBCSP) and Community Networks Program (CNP) cancer education and outreach programs.

Community Networks Program (CNP) - The Community Networks Program, a collaborative initiative with Meharry Medical College, is designed to reduce and eliminate cancer health disparities through community based participatory research, education and training. The CNP focuses on five specific types of cancers: Breast, Cervical, Colorectal, Lung and Prostate and targets African American men and women over 21 years of age.

Tennessee Breast and Cervical Cancer Early Detection and Prevention Program (TBCSP) - The Tennessee Breast and Cervical Cancer Early Detection and Prevention program is designed to provide cancer screening and education services to eligible women. The program offers services to those women who meet the following requirements:

a resident of Tennessee;
at or below 250% of the Federal Poverty Level (FPL) for family size;
uninsured or underinsured;
age requirements for breast screening – primary target group, women 50 – 64 yrs of age;
secondary target group, women 40 – 49 yrs of age;
age requirements for cervical screening – primary target group, women 40-64;
secondary target group, women 18 – 39.

Social Services - MHC employs a licensed social worker who assists patients with access to supportive services that include: case management, referrals for mental health and substance abuse, and other available services that can positively impact the cultural, social and economical status of our patient population. The social worker also facilitates the Patient Assistance Program which gives clients without prescription coverage access to medications for free or at a nominal fee, based on meeting income criteria. The social worker also makes referrals for mental health and substance abuse services.

Healthcare for the Homeless (HCH) - The Healthcare for the Homeless program is designed to provide primary and preventive healthcare services to homeless individuals in Memphis and Shelby County. The HCH program is open to any person determined to be homeless and in need of supportive services. Program participation requires documentation confirming homeless status. Acceptable forms of verification include:

letter submitted by an approved/identified agency servicing homeless individuals;
recent release letter from correctional institution;
notarized letter from individual providing temporary shelter (7-60 days);
proof of income, such as: check stub, social security determination letter, or unemployment statement if not currently residing in a shelter or recently released from a correctional institution. A picture ID is required to receive service.

Ryan White Part A (HIV/AIDS) - The Ryan White HIV/AIDS program is designed to provide medical care, confidential counseling, community and social service resources to HIV/AIDS infected persons. The program is open to all uninsured persons who are infected with the HIV/AIDS virus and in need of primary care, case management and other supportive services.

Ryan White HIV Prevention: Healthy Relationships - Healthy Relationships is a five-session, small-group intervention for men and women living with HIV/AIDS. It is based on Social Cognitive Theory and focuses on developing skills and building self-efficacy and positive expectations about new behaviors through modeling behaviors and practicing new skills. W.I.C (Women, Infants & Children) - The W.I.C. program is designed to provide supplemental food and nutrition education that promotes good health for pregnant, breastfeeding and postpartum women, infants and children up to age five. The program is open to pregnant women, women who have a baby less than six months old, those of childbearing age, and children and infants who are at nutritional risk, live in Shelby County and meet liberal income guidelines. Participation requirements include proof of the following at each certification visit: income, identity of person receiving vouchers, residency, and shot record for infants and children.

340b Reduced Price Prescription Drug Program - Thanks to Memphis Health Centers' status as a federally qualified health center through the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA), our on-site pharmacy is able to offer our patients free and reduced price prescription medications under the 340b Drug Program. Section 340b of the Social Security Act implemented limitations on the cost of covered outpatient drugs to certain federal grantees. Participation in the Program results in significant savings estimated to be 20% to 50% on the cost of pharmaceuticals. The purpose of the 340b Program is to reach more eligible patients and provide more comprehensive services to medically under-served communities.
